GROUP PROGRAM SCHEDULE: Weekly gestational diabetes management class
PHYSICIAN REFERRAL REQUIRED: Yes (not for Senior Wellness)

SCREENING SERVICES: Blood glucose testing provided on a walk-in or appointment
basis at senior centers in Grant County; also provided at annual health
fairs and county fair.
TRANSPORTATION: Hospital van available if needed for gestational diabetes
classes.
OTHER: Senior Wellness program participants must be 55 years of age or older.
