A Note To Providers O' Diabetes
Education Services

This is the second edition of a directory that will be published every 2 years and is made available throughout New Mexico at no charge.

To List in the Directory: We encourage any providers whose services we have inadvertently missed to mail in the "Request for Directory Listing" form at the back of the directory so you can be included in the next edition.

Address Changes: If you are listed in the current directory and your address has changed, please fill out the "Change of Address" form at the back in order to receive future directory listing questionnaires.

Next Edition: This directory will be published again in the Spring of 1997. Directory listing questionnaires will be mailed in Winter 1996.

To Order Directories

For additional free copies of the directory, please fill out the "Directory Order Form" at the back. It is our hope that physicians, dietitians, nutritionists, nurses, health educators, pharmacists, the staff of senior citizen and community centers, people with diabetes and others will make these directories available to their clients, patients and friends.

Team Up With Your Doctor

Having diabetes means that you are more likely to have problems with your eyes, heart, kidneys, nerves and skin than other people. You can take care of your diabetes and keep yourself healthy by becoming "partners" with your doctor or other health care provider.

At each visit, your doctor should:

(INDENT)- Blood Sugar (blood glucose)
- Hemoglobin A-one-C(INDENT)

Once a year, your doctor should:
Regular eye exams are very important for detecting
diabetic eye disease and preventing vision problems and or even blindness.

If your doctor forgets any of these exams,
be sure to remind him or her!
